Antecedentes: La pandemia de COVID-19 en Guatemala provocó el rebalse de los ya colapsados hospitales. Los servicios de otras especialidades además de Medicina Interna tuvieron que emplearse de lleno en atender la pandemia. En el Hospital General San Juan de Dios, el servicio de operados de emergencia atendió durante 4 meses pacientes con esta enfermedad, a cargo del personal quirúrgico, residentes de infectología y de medicina de emergencias para apoyar a la especialidad de Medicina Interna; mostrando la necesidad de la intervención de todas las especialidades en la atención a la pandemia. Material y Métodos: Estudio descriptivo transversal sobre la experiencia del servicio de operados de Emergencia en la atención a pacientes con enfermedad COVID-19 de julio a octubre del 2020. Resultados: Se atendieron 178 pacientes con COVID-19, con predominio de sexo masculino 57%, con edades de 16 a 91 con una media de 65 años.70 pacientes eran hipertensos, 69 diabéticos y 13 con enfermedad renal. 169 casos tuvieron neumonía por SARS-COV2, 2 con choque y fallo orgánico múltiple. La mortalidad fue de 13 casos (7 %). Los medicamento más utilizados fueron las heparinas de bajo peso molecular. El 86 % recibió terapia con esteroides, 50% con anticuerpos monoclonales, especialmente tocilizumab. El antibiótico más usado fue la Ceftriaxona (32 %). 6 pacientes recibieron remdesivir (3%). CONCLUSIONES: Este estudio evidencia la experiencia del servicio de operados de emergencia durante la pandemia de COVID19, donde médicos no expertos en enfermedades virales infecciosas, aprendieron sobre la marcha el manejo de esta enfermedad. Un significativo número de pacientes fueron beneficiados con la implementación de estos servicios. El aprendizaje, las competencias y los servicios que presta el médico dedicado a urgencias debe adaptarse para responder a las necesidades de la población (AU)


Background: The COVID-19 pandemic in Guatemala collapsed hospitals due to overflow. The services of other specialties in addition to Internal Medicine had to be fully used to attend the pandemic. At the San Juan de Dios General Hospital, the emergency surgery service treated patients with this disease for 4 months, in charge of surgical personnel, infectology residents and emergency medicine support the Internal Medicine specialty; showing the need for the intervention of all specialties in the care of the pandemic. METHODOLOGY: Cross-sectional descriptive study on the experience of the Emergency Service in caring for patients with COVID-19 disease from July to October 2020. Results: A total of 178 patients with COVID-19 were treated, with a male predominance of 57%, aged from 16 to 91 with an average of 65 years. In addition to COVID-19 disease, 70 patients were hypertensive, 69 diabetics, and 13 with kidney disease. 169 cases had SARS-COV2 pneumonia, 2 with shock and multiple organ failure. Mortality was 13 cases (7%). Because most of the cases required oxygen and were moderate, severe and severe, drug treatment was used with high frequency. The drug that was used the most was heparins in all cases, most of them low molecular weight. 86% received steroid therapy, 50% with monoclonal antibodies, especially tocilizumab. Antibiotics were used frequently, the majority (32%) received Ceftriaxone. 6 cases received remdesivir (3%). CONCLUSIONS: The COVID-19 pandemic has demanded that doctors from all specialties join the response and fight against this disease. This study shows that all specialties have the capacity to handle patients with COVID-19 since internal medicine and related services have exceeded their capacity to care (AU)

ABSTRACT

Existen múltiples patologías del tracto urinario, ya sea congénitas o adquiridas, que requieren de tratamiento quirúrgico de Nefrectomía total o parcial. En el siguiente estudio longitudinal prospectivo se incluyen 21 nefroureterectomías en pacientes pediátricos con patología del tracto urinario con o sin enfermedad renal crónica y algún tipo de terapia de reemplazo renal, realizados con la técnica de Cirugía Minimamente Invasiva (CMI) abordaje retroperitoneal o retroperitoneoscopía. There are several urinary tract diseases, such as congenital or adquired, that require total or partial nephrectomy. This prospective longitudinal study included 21 total nephrectomies in pediatric patients with or without end stage renal disease and some kind of renal replacement therapy. All surgeries were done with retroperitoneal approach using minimally invasive surgery (MIS), retroperitoneoscopy. (AU)

ABSTRACT

Objective:To analyze how the literature presents the quality of life in renal transplant patients. Methods: base review study: MEDLINE, LILACS and SCIELO. The descriptors kidney Transplantation and quality of life were used, combined with the boolean operator and. After considering the criteria, 15 articles were fully analyzed.Results: the publications showed that the transplantation contributes to the improvement of the patient's quality of life, although, compared to the general population, this quality is lower. Factors such as: presence of comorbidities, transplantation time, educational level and psychological aspects influence the patient's view of their quality of life.Conclusion: the quality of life after transplantation is surrounded by multiple aspects and particularities. Finding the benefits of transplantation for the renal patient does not exempt the possibility of negative dimensions that may compromise the perception of quality of life


Objetivo: Analisar como a literatura apresenta a qualidade de vida em paciente transplantado renal. Métodos: estudo de revisão nas bases: MEDLINE, LILACS e SCIELO. Utilizou-se os descritores "Kidney Transplantation" e "Quality of Life", combinando-se com o operador booleano AND. Após consideração dos critérios, foram analisados integralmente 15 artigos. Resultados: As publicações evidenciaram que o transplante contribui para melhoria da qualidade de vida do paciente, ainda assim, se comparado à população geral essa qualidade é inferior. Fatores como: presença de comorbidades, tempo de transplante, nível de escolaridade e aspectos psicológicos influenciam a visão que o paciente possui da sua qualidade de vida. Conclusão: a qualidade de vida após o transplante é envolta de múltiplos aspectos e particularidades. A constatação dos benefícios do transplante para o doente renal, não isenta a possibilidade da existência de dimensões negativas as quais podem comprometer a percepção da qualidade de vida.

Resultados: las publicaciones mostraron que el trasplante contribuye a la mejora de la calidad de vida del paciente, aunque, en comparación con la población general, esta calidad es menor. Factores como: presencia de comorbilidades, tiempo de trasplante, nivel educativo y aspectos psicológicos influyen en la visión del paciente sobre su calidad de vida.Conclusión: la calidad de vida después del trasplante está rodeada de múltiples aspectos y particularidades. Encontrar los beneficios del trasplante para el paciente renal no exime la posibilidad de dimensiones negativas que pueden comprometer la percepción de la calidad de vida

ABSTRACT Objectives This study aims to improve laparoscopic nephrectomy techniques for inflammatory renal diseases (IRD) and to reduce complications. Materials and Methods Thirty-three patients underwent laparoscopic nephrectomy for IRD, with a method of outside Gerota fascia dissection and en-bloc ligation and division of the renal pedicle. Operative time, blood loss, complications, analgesia requirement, post-operative recovery of intestinal function and hospital stay were recorded. The degrees of perinephric adhesion were classified based on the observation during operation and post-operative dissection of the specimen, and the association of different types of adhesion with the difficulty of the procedures was examined. Results Among 33 cases, three were converted to hand-assisted laparoscopy, and one was converted to open surgery. Mean operative time was 99.6±29.2min, and blood loss was 75.2±83.5 mL. Postoperative recovery time of intestinal function was 1.6±0.7 days and average hospital stay was 4.8±1.4 days. By classification and comparison of the perinephric adhesions, whether inflammation extending beyond Gerota fascia or involving renal hilum was found to be not only an important factor influencing the operative time and blood loss, but also the main reason for conversion to hand-assisted laparoscopy or open surgery. Conclusions In laparoscopic nephrectomy, outside Gerota fascia dissection of the kidney and en-bloc ligation of the renal pedicle using EndoGIA could reduce the difficulty of procedure and operative time, with satisfactory safety and reliability. Inflammation and adhesion extending beyond Gerota fascia or involving renal hilum is an important predictor of the difficulty related to laparoscopic nephrectomy for IRD.

ABSTRACT Introduction and objective Urethral duplication is a rare congenital anomaly, with roughly 200 cases reported in the literature (1). It is more frequent in males, with few cases reported in females. The clinical presentation differs according to the anatomical variant present. The duplication most commonly occurs in the sagittal plane with one urethra located ventrally and the other dorsally (2). Usually the ventral urethra is the more functional of both. Duplications occurring in the coronal plane are quite rare and they are usually associated with bladder duplication (3). The purpose of this paper was to present a video of a boy with an unusual urethral duplication form. Materials and Methods Patient was born premature due to oligohydramnios at 7 months-gestational age and he has initial diagnosis of hypospadia. Since then, he presented at least 7 febrile UTI and mother complained of difficult micturition and a presence of a mass at lower abdomen. Patient was referred to our institution and we identified urethral duplication with a glandar and scrotal meatus, palpable bladder and left penile-hemiscrotum transposition. US and CT-scan showed left kidney agenesis and overdistended bladder. VCUG and retrograde urethrography showed duplication, presence of contrast in the seminal vesicles and complete catheterizing of both urethras was not possible. Results The topic urethra was dysplastic and not patent to a 4Fr plastic tube so we were unable to access it endoscopically. We performed initially a Mitrofanoff procedure to allow CIC and treat chronic retention. Six months later, we assessed both urethras surgically and concluded that dorsal urethra was dysplastic after 3cm still in the penile area and scrotal urethra was not possible to be catheterized. We excised the ventral urethra because of dribbling complaints up to bulbar area and reconstructed the scrotal transposition, keeping the topic urethra for cosmetic issues. Patient had excellent outcome, performs CIC every 4 hours and has not presented further UTI episodes. Discussion and conclusion The urethral duplication is an anomaly that has multiple anatomical presentations. There are several theories about the etiology, but none can explain all types of presentations. There is also more than one rating available, and the Effmann classification is the most detailed. The case exemplifies this varied spectrum of anatomic urethral duplication. It resembles the urethral duplication type IIa-Y, however, ventral urethra meatus was located in penoscrotal area and both urethras were at least partially hypoplastic/dysplastic associated with obstruction and bladder retention. In determining how to best manage a patient with Y-type urethral duplication, the caliber and quality of the orthotopic urethra must first be assessed. Published reports suggest that best outcomes are those using the ventral duplicated urethra for the reconstruction (4). In this case, none of urethras were functional and a supravesical outlet channel had to be provided. The treatment of this condition requires an individualized planning and a vast technical knowledge of reconstructive surgery.

ABSTRACT

El tumor fibroso solitario es raro. Si bien su localización más frecuente es la pleura, han sido informados algunos casos de ubicación retroperitoneal. Es difícil diferenciarlo imagenológicamente de otras neoplasias, por lo que casi siempre el diagnóstico es histológico. Dado que los tumores fibrosos solitarios tienen presentaciones clínicas muy diversas, una mejor compresión de la ubicación y sus características imagenológicas ayudaría a abreviar la lista de diagnósticos diferenciales. Comunicamos un tumor fibroso solitario de localización retroperitoneal cuyo diagnóstico fue confirmado por examen histológico.


The solitary fibrous tumor is uncommon. Even though it frequently locates in the pleura, a few cases have been reported in the retroperitoneum. Differentiation from other neoplasms is difficult, and therefore the diagnosis is always attained through histological examination. Although solitary fibrous tumors have variable clinical behaviors, a better comprehension of the location and its imaging characteristics would help to decrease the list of differential diagnoses. We report a solitary fibrous tumor located in the retroperitoneum whose diagnosis was confirmed by histological examination.

ABSTRACT

Chronic kidney disease affects approximately 10% of the world's adult population: it is within the top 20 causes of death worldwide, and its impact on patients and their families can be devastating. World Kidney Day and International Women's Day in 2018 coincide, thus offering an opportunity to reflect on the importance of women's health and specifically women's kidney health on the community and the next generations, as well as to strive to be more curious about the unique aspects of kidney disease in women so that we may apply those learnings more broadly. Girls and women, who make up approximately 50% of the world's population, are important contributors to society and their families. Gender differences continue to exist around the world in access to education, medical care, and participation in clinical studies. Pregnancy is a unique state for women, offering an opportunity for diagnosis of kidney disease, but also a state in which acute and chronic kidney diseases may manifest, and which may impact future generations with respect to kidney health. Various autoimmune and other conditions are more likely to impact women, with profound consequences for child bearing and the fetus. Women have different complications on dialysis than men, and are more likely to be donors than recipients of kidney transplants. In this editorial, we focus on what we know and do not know about women, kidney health, and kidney disease, and what we might learn in the future to improve outcomes worldwide.

SUMMARY Introduction: Obstructive nephropathy is a frequent complication in the course of advanced cervical cancer (CC), and ultrasonography-guided percutaneous nephrostomy (PCN) is a well established technique for fast ureteral desobstruction. Objective: To identify possible factors related to the survival and quality of life of patients with advanced CC presenting acute urinary obstructive complications that after desobstruction by PCN recovered urinary flux and renal function. Method: This is an analytical, descriptive, cross-sectional study that included 45 patients with CC who underwent PCN and were divided into 2 groups: “death” (DG) and “survival” (SG), in a public hospital that is reference for oncologic diseases in Northern Brazil. Results: The mean serum creatinine of the patients preceding PCN was >10 mg/dL, and after PCN <2 mg/dL. The cutoffs of 8.7 g/dL for Hb (p=0.0241) and 27% for Ht (p=0.0065) indicated the values that better discriminate the outcomes of the groups. The presence of low blood pressure was statistically correlated (p=0.0037) to the outcome “death”. Changes in glomerular filtration rate (already reduced in all cases) were not associated to the levels of Hb/Ht or to the outcome “death” during the nephrological follow-up. Conclusion: PCN was responsible for the recovery of renal function in 61.7% of the patients, leading to interruption of renal replacement therapy (RRT) in all of those patients. Hb levels >8.7g/dL and Ht >27% were associated to longer survival, and the presence of low blood pressure during follow-up was associated with progression to death.


RESUMO Introdução: a nefropatia obstrutiva é complicação frequente na evolução do câncer do colo uterino (CCU) avançado e a nefrostomia percutânea guiada por ultrassonografia (NFT) é uma técnica bem estabelecida para a rápida desobstrução ureteral. Objetivo: esclarecer os fatores relacionados à evolução ou não para óbito e qualidade de vida das pacientes com CCU avançado com complicações obstrutivas urinárias agudas e que, após desobstrução pela NFT, recuperaram fluxo urinário e função renal. Método: foi realizado estudo transversal analítico descritivo, que avaliou dois grupos de pacientes com CCU submetidas à NFT [óbito (GO) e sobrevida (GS)], em um hospital público, referência para doenças oncológicas da região Norte do país. Resultados: a creatinina sérica média inicial era >10 mg/dL pré-NFT e tornou-se <2 mg/dL após. Quanto à sobrevida, os pontos de corte de 8,7 g/dL de Hb e 27% de Ht melhor discriminaram a evolução dos grupos GO e GS (p=0,0241 e p=0,0065). Hipotensão se associou significantemente (p=0,0037) com a evolução para óbito. Variações na taxa de filtração glomerular, que já era reduzida em todos os casos, não se associaram aos níveis de Hb/Ht ou à evolução para óbito durante seguimento nefrológico. Conclusão: a NFT permitiu a recuperação da função renal em 61,7% das pacientes com CCU, dispensando terapia de substituição renal. Níveis de Hb >8,7 g/dL e Ht >27% estiveram associados a maior sobrevida, e a hipotensão durante o seguimento associou-se com evolução para óbito.

OBJECTIVE: Chronic rejection remains a major cause of graft failure with indication for re-transplantation. The incidence of chronic rejection remains high in the pediatric population. Although several risk factors have been implicated in adults, the prognostic factors for the evolution and reversibility of chronic rejection in pediatric liver transplantation are not known. Hence, the current study aimed to determine the factors involved in the progression or reversibility of pediatric chronic rejection by evaluating a series of chronic rejection cases following liver transplantation. METHODS: Chronic rejection cases were identified by performing liver biopsies on patients based on clinical suspicion. Treatment included maintaining high levels of tacrolimus and the introduction of mofetil mycophenolate. The children were divided into 2 groups: those with favorable outcomes and those with adverse outcomes. Multivariate analysis was performed to identify potential risk factors in these groups. RESULTS: Among 537 children subjected to liver transplantation, chronic rejection occurred in 29 patients (5.4%). In 10 patients (10/29, 34.5%), remission of chronic rejection was achieved with immunosuppression (favorable outcomes group). In the remaining 19 patients (19/29, 65.5%), rejection could not be controlled (adverse outcomes group) and resulted in re-transplantation (7 patients, 24.1%) or death (12 patients, 41.4%). Statistical analysis showed that the presence of ductopenia was associated with worse outcomes (risk ratio=2.08, p=0.01). CONCLUSION: The presence of ductopenia is associated with poor prognosis in pediatric patients with chronic graft rejection.

ABSTRACT

Hydatid cyst is a parasitic disease caused by the tapeworm Echinococcus granulosus. Hydatid cyst disease mostly involves the liver and the lung, while renal involvement is rare. Hydatid disease may present in unusual ways and lead to diagnostic diffi culty and management problems. A high index of suspicion for hydatid disease should be maintained while evaluating complex cystic renal masses. Herein we present an interesting case who was diagnosed to have a simple renal cyst on contrast-enhanced computed tomogram, which turned out to be a renal hydatid cyst. Despite modern imaging methods, isolated renal hydatid disease might still cause diagnostic dilemma and hydatid cysts can be found in unusual localization. Th is case was managed laparoscopically.

ABSTRACT

Objetivo: Analisar as características do doador de múltiplos órgãos, incidência e duração da função retardada do enxerto (FRE), e seu impacto na função renal no primeiro ano após o transplante. Métodos: Foi realizado um estudo retrospectivo, unicêntrico, observacional, analisando os transplantes renais com doador falecido realizados em 2010 no nosso serviço. Resultados: A taxa de FRE foi de 68%, com mediana de duração de 12 dias (variação, 1-61 dias). Quarenta e quatro (38%) pacientes apresentaram FRE com 12 ou mais dias de duração (FRE prolongada). A idade média dos doadores foi de 43 ± 13 anos e 37% deles eram hipertensos. Em 59% dos doadores, a causa da morte foi acidente cerebrovascular, e o tempo de isquemia fria (TIF) médio foi de 23 ± 5 horas. Os receptores tinham idade média de 51 ± 15 anos, tempo em diálise de 43 meses (variação, 1-269) e 25% eram sensibilizados (PRA > 0%). No modelo de regressão logística multivariada, a presença de vasculopatia na biópsia de captação foi o único fator de risco independente para o desenvolvimento de FRE prolongada [OR 3,6 IC 95% (1,2-10,2), p = 0,02]. Os pacientes com FRE prolongada apresentaram pior função renal 1 ano após o transplante em comparação com os pacientes sem FRE (SCr 1,7 vs. 1,3 mg/dL, respectivamente, p = 0,03). Conclusão: A presença de vasculopatia na biópsia de captação foi identificada como fator de risco independente para o desenvolvimento de FRE prolongada. A FRE prolongada foi associada com pior função renal no 1º ano após o transplante. .


Objective: The purpose of this study was to evaluate the impact of donor and recipient characteristics on duration of delayed graft function (DGF) and 1-year serum creatinine (SCr), as a surrogate endpoint for allograft survival. Methods: We reviewed 120 first cadaver kidney transplants carried out consecutively at our center to examine the effect on 1-year SCr of the presence and duration of DGF. Results: DGF rate was 68%, with a median duration of 12 days (range, 1-61). Forty-four (38%) patients presented DGF lasting 12 or more days (prolonged DGF group). Mean donor age was 43 ± 13 years, 37% had hypertension and in 59% the cause of brain death was cardiovascular accident. The mean cold ischemia time was 23 ± 5 hours. Twenty-seven (23%) donors were classified as expanded-criteria donors according to OPTN criteria. The mean recipient age was 51 ± 15 years. The recipients median time in dialysis was 43 months (range, 1-269) and 25% of them had panel reactive antibodies > 0%. Patients with prolonged DGF presented higher 1-year SCr in comparison with patients without DGF (1.7 vs. 1.3 mg/dL, respectively, p = 0.03). In multivariate logistic regression analysis, the only significant factor contributing to the occurrence of prolonged DGF was the presence of vascular lesions in the kidney allograft at time of transplantation (HR 3.6, 95% CI 1.2-10.2; p = 0.02). Conclusion: The presence of vasculopathy in the kidney allograft at time of transplantation was identified as an important factor independently associated with prolonged DGF. Prolonged DGF negatively impacts 1-year graft function. .

ABSTRACT

Renal subcapsular abscess is a very rare entity that is defined by a suppurative process localized to a space between the renal capsule and the renal parenchyma. The pathogenesis and aetiology of this entity remain speculative. To our knowledge, only five cases have been reported in the English literature. We describe a 74-year old woman with renal subcapsular abscess treated with laparoscopic removal and do a review of the literature.


El absceso renal subcapsular es una entidad muy rara que se define por un proceso supurativo localizado en un espacio entre la cápsula renal y el parénquima renal. La patogénesis y la etiología de esta entidad siguen siendo asunto de especulación. Hasta donde sabemos, solamente cinco casos han sido reportados en la literatura inglesa. ABSTRACT

The laparoscopic approach has been established as the surgical procedure of choice for radical nephrectomy during the recent years. The advantages of the laparoscopic radical nephrectomy in comparison to the open approach are well-documented. The oncological results of the laparoscopic approach are similar to the open procedure while the post-operative morbidity is lower. Laparoscopic partial nephrectomy seems to gain ground to its open counterpart, as the accumulation of experience in the technique grows. In this review, a PubMed search in the latest literature on radical and partial laparoscopic nephrectomy took place and the outcome of the search is presented. Several issues about the surgical techniques and clinical efficacy are discussed. In addition, the preliminary experience in laparoscopic nephrectomy of one of the authoring institutions is also presented

OBJECTIVE: To evaluate the safety and efficacy of transarterial nephrectomy, i.e., complete renal artery embolization, as an alternative to surgical nephrectomy. MATERIALS AND METHODS: This retrospective study included 11 patients who underwent transarterial nephrectomy due to a high risk of surgical nephrectomy or their refusal to undergo surgery during the period from April 2002 to February 2013. Medical records and radiographic images were reviewed retrospectively to collect information regarding underlying etiologies, clinical presentations and embolization outcomes. RESULTS: The underlying etiologies for transarterial nephrectomy included recurrent hematuria (chronic transplant rejection [n = 3], arteriovenous malformation or fistula [n = 3], angiomyolipoma [n = 1], or end-stage renal disease [n = 1]), inoperable renal or ureteral injury (n = 2), and ectopic kidney with urinary incontinence (n = 1). The technical success rate was 100%, while clinical success was achieved in eight patients (72.7%). Subsequent surgical nephrectomy was required for three patients due to an incomplete nephrectomy effect (n = 2) or necrotic pyelonephritis (n = 1). Procedure-related complications were post-infarction syndrome in one patient and necrotic pyelonephritis in another patient. Of four patients with follow-up CT, four showed renal atrophy and two showed partial renal enhancement. No patient developed a procedure-related hypertension. CONCLUSION: Transarterial nephrectomy may be a safe and effective alternative to surgical nephrectomy in patients with high operative risks.

INTRODUCCIÓN: La hidatidosis es una parasitosis endémica en Chile. Los órganos más frecuentemente afectados son hígado y pulmón. Otras localizaciones, tales como la renal son infrecuentes e implican dificultades diagnósticas. PRESENTACIÓN DEL CASO: Mujer de 25 años, sin antecedentes mórbidos, que consulta por dolor lumbar de un mes de evolución, hematuria autolimitada y distensión abdominal, sin hallazgos patológicos al examen físico En sus estudios de laboratorio realizados en Hospital Base de Puerto Montt, Chile, destaca leucocitosis de 13.500 células/mm3 con eosinofilia relativa de 31,4 por ciento, Velocidad de Eritrosedimentación de 74 mm/h y función renal normal. Se solicita ecotomografía abdominal, en la cual se encuentra un quiste renal izquierdo complejo, hallazgo complementado con Tomografía Axial Computada abdomino-pélvico, la cual confirma quiste renal izquierdo de 13 centímetros de diámetro sin aspecto tumoral y quiste en fosa ilíaca izquierda en posición paravesical e hígado sin lesiones. Adicionalmente se realiza radiografía de tórax, la que resulta de aspecto normal. Se realiza Test para Hidatidosis que resulta positivo. Posteriormente, ante la sospecha de hidatidosis renal, se inicia tratamiento con Albendazol 400 mg al día durante 45 días y posteriormente quistectomía renal, evolucionando de forma satisfactoria. Se decide diferir cirugía de quiste paravesical. DISCUSIÓN: La localización renal de un quiste hidatídico es infrecuente y corresponde a menos del 2 por ciento de los casos, el diagnóstico de un quiste complejo renal debe considerar como diagnóstico diferencial la posibilidad de un quiste hidatídico.


INTRODUCTION: Hydatidosis is an endemic parasitic disease in Chile. The main organs affected are the liver and lungs. Other locations, such as the kidneys, are infrequent and involve diagnostic difficulties. CASE REPORT: A 25 year old woman with no previous history of morbid diseases consulting for a one month evolution of lumbar pain, self-limited hematuria and abdominal distension, with no abnormaities on physical examination. In her laboratory studies there was leukocytosis of 13,500 cells/mm3 with relative eosinophilia of 31.4 percent erythrocyte sedimentation rate (ESR) of 74 mm/h and normal renal function. Abdominal ultrasound found a complex left renal cyst, which was complemented with an abdomen and pelvis computerized axial tomography (CT), which confirmed a 13-centimeter left renal cyst without tumor-like appearance. Additionally, chest x-ray shows normal appearance. Enzyme-linked immunosorbent assay (ELISA) for hydatidosis is performed, which results positive. Subsequently, and based on the renal hydatid disease suspicion, she was treated with Albendazole 400 mg per day for 45 days followed by renal cystectomy, evolving satisfactorily. It was decided to defer paravesical cyst surgery. DISCUSSION: Renal hydatid cysts are rare and correspond to less than 2 percent of all cases. The diagnosis of kidney cyst should consider the hydatid cyst as a differential diagnosis.

El trauma renal es una patología que se presenta con frecuencia en pacientes politraumatizados. Los accidentes de tránsito son la causa más frecuente de trauma renal, debido principalmente al impacto contuso que reciben las estructuras renales. El trauma penetrante es menos frecuente, aunque con mayor frecuencia causa lesiones graves que requieren de cirugía para su resolución. La evaluación inicial del estado hemodinámico de estos pacientes en el momento de ingreso hospitalario es el gran determinante para definir la conducta terapéutica a seguir: los pacientes que ingresen estables pueden ser manejados conservadoramente obteniendo buenos resultados y los pacientes inestables deben ser explorados quirúrgicamente con el fin de descartar lesiones renales graves que sean causa de su inestabilidad. En los últimos años, los centros de trauma han adoptado conductas de manejo conservador en las lesiones traumáticas de órganos sólidos, basadas principalmente en el uso de imágenes (Ecotomografía de Urgencia y Tomografía Computarizada de abdomen)para determinar la presencia de lesiones o líquido libre intra-abdominal, lo que permite diferenciar con mayor certeza los pacientes que requieren cirugía de urgencia. Si bien las lesiones renales graves son las menos frecuentes, en el contexto de un paciente politraumatizado que ingrese hemodinámicamente inestable, se debe tener una alta sospecha y descartarlas en el menor tiempo posible. Las complicaciones son infrecuentes, siendo la extravasación urinaria la de mayor presentación y en la mayoría de los casos revierte espontáneamente.


Renal trauma is a condition that occurs frequently in trauma patients. Motor vehicle accidents are the most common cause of renal trauma, mainly due to the blunt impact that kidney receives. Penetrating trauma is less common, but most often causes serious injuries that require surgery. The initial evaluation of the hemodynamic status of these patients is the major determinant in defining the proper course of action: stable patients can be managed conservatively with good results and unstable patients should be surgically explored to rule out severe renal lesions that may cause the instability. In recent years, trauma centers have adopted conservative behavior in traumatic injuries of solid organs, mainly based on the use of images (abdominal Ultrasoundand Computerized Tomography) to determine the presence of injuries or intra-abdominal free fluid, allowing to differentiate patients that require emergency surgery or not. Even though severe renal lesions are less frequent, in the context of a hemodynamically unstable trauma patient, these lesions should be suspected and ruled out promptly. Complications are infrequent, being urinary extravasation the more prevalent and in most cases reverts spontaneously.

Despite significant advances in laparoscopic technique and technologies, laparoscopic Urologic surgery remains technically demanding regarding various surgical steps including the challenge of specimen retrieval and extraction, whether to install a drainage system and the best option for wound closure. Laparoscopic specimen entrapment and extraction occurs at what is falsely considered the "end of the procedure". During open surgery, after the specimen has been mobilized, the specimen is simply lifted out of the larger incision which has been made to achieve the surgical objectives. In contrast, significant laparoscopic skill is required to entrap and safely extract laparoscopic specimens. Indeed, the Urologist and surgical team which are transitioning from open surgery may disregard this important part of the procedure which may lead to significant morbidity. As such, it is imperative that during laparoscopic procedures, the "end of the procedure" be strictly defined as the termination of skin closure and dressing placement. Taking a few minutes to focus on safe specimen entrapment and extraction will substantially reduce major morbidity. The following review focus on the technology and technique of specimen entrapment and extraction, on the matter of whether to install a drainage system of the abdominal cavity and the options for adequate closure of trocar site wounds. This article's primary objectives are to focus on how to minimize morbidity while maintain the advantages of a minimally invasive surgical approach.

Introduction: Minimally invasive surgery in urology is rapidly advancing and Laparo-endoscopic single-site surgery (LESS) is not the exception. Such laparoscopic procedures are technically challenging and require an experienced laparoscopic surgeon due to the lack of port placement triangulation and instrument clashing. The benefit of the da Vinci surgical system has recently introduced to LESS. We present two cases of robotic LESS nephrectomy. Matherials and Methods: Two patients, a female of 23 years old, diagnosed with right renal atrophy secondary to chronic pyelonephritis and one male patient with diagnosis of left staghorn calculi and renal atrophy. Both underwent to a total nephrectomy assisted by the da Vinci S surgical system through a single port incision using the GelPoint® access system. Results: The first surgery was performed without incidents or conversion. The second patient required the installation of an additional robotic port for triangulation. The dock time and the mean operative time was 18 and 110 min. The mean estimated blood loss was 100 cc and the hospital stay was 27 hours. There were no complications. Conclusions: LESS robotic surgery is feasible using current robotic systems. However, there are several limitations. The design of specific technology for the use of the robot through single incision can solve this problem.


Introducción: La cirugía mínimamente invasiva en urología avanza rápidamente y la cirugía laparo-endoscópica a través de puerto único (LESS) no es la excepción. Esta técnica por vía laparoscópica presenta mucha dificultad y requiere de un cirujano laparoscópico experimentado debido a la falta de triangulación y el cruce de los instrumentos. Los beneficios del sistema quirúrgico da Vinci® han sido introducidos recientemente en LESS. Presentamos dos casos de nefrectomía LESS asistida por robot. Materiales y Métodos: Dos pacientes, la primera de sexo femenino y 23 años de edad, con diagnóstico de atrofia renal secundaria a pielonefritis crónica derecha y un segundo paciente de sexo masculino con diagnóstico de litiasis coraliforme izquierda y atrofia renal. Ambos pacientes fueron sometidos a una nefrectomía total asistida por el sistema quirúrgico robótico da Vinci S a través de un puerto único utilizando el dispositivo de acceso GelPoint®. Resultados: La primera cirugía fue llevada a cabo sin incidentes, sin necesidad de conversión. En el segundo paciente fue necesaria la instalación de un puerto robótico adicional para triangulación. El tiempo de acoplamiento y operatorio promedio fue de 18 y 110 min. El sangrado promedio estimado fue de 100 cc y la estadía hospitalaria promedio de 27 horas. No hubo complicaciones. Conclusiones: La cirugía robótica LESS es posible de realizar utilizando los sistemas robóticos actuales. Sin embargo, existen diversas limitaciones. El diseño de tecnología específica para el uso del robot por puerto único podrá solucionar este problema.

ABSTRACT

PURPOSE: To assess the clinical utility of laparoendoscopic single-site surgery (LESS) nephrectomy using a modified umbilical incision and home-made transumbilical port in cases requiring extirpative surgery. MATERIALS AND METHODS: Initial consecutive 18 patients underwent LESS nephrectomies that were performed by a single surgeon. A home-made port was placed through a modified umbilical incision, the length of which had preoperatively been determined. The laparoscopic transperitoneal nephrectomy procedures were performed with various combinations of standard and articulating laparoscopic instruments. Patient characteristics and perioperative outcomes, including pathologic results, were recorded prospectively. RESULTS: All 18 extirpative surgeries, including simple nephrectomy (eight cases), radical nephrectomy (nine cases), and nephroureterectomy (one case), were completed successfully. The median operation time was 167 min (range 82-220), and the median blood loss was 250 mL (range 0-1050). All specimens were extracted intact through a modified umbilical incision (median length 2.5 cm, range 1.5-6.0). Final pathological analysis revealed a nonfunctioning kidney in five cases, a dysplastic kidney in three cases, a mixed epithelial and stromal tumor in one case, renal cell carcinoma in eight cases (T1: five cases, T3: three cases), and Ta ureter transitional cell carcinoma in one case. CONCLUSION: LESS nephrectomy using a home-made port and modified umbilical incision is feasible with both minimal incision and cost-effective. Our technique may be more useful for extirpative procedures in which a specimen needs to be removed intact, because incision length can be freely adjusted. Prospective comparisons are warranted to more clearly elucidate the utility of this surgical technique.

Objectives: Surgical treatment of "Ren Mobilis" has historically been associated with poor results and fairly high morbidity. We have used a transperitoneal laparoscopic approach in order to minimize morbidity. The goal of this study was to evaluate the success rate and to discuss the possible pathogenic mechanism, which has implications for the surgical strategy. Materials and methods: Seven women with a right mobile kidney were examined by intravenous pyelogram and CT scans. Symptoms were judged to emanate from the mobile kidney. Transperitoneal laparoscopic nephropexy was performed. The surgical treatment consisted of fixing the kidney to the dorsal abdominal wall using tissue glue (Tisseel®) after diathermy coagulation of the surfaces to induce fibrosis. The right colon was fixed with clips to the lateral abdominal wall, trapping the kidney in place. Results: In 6 of the cases, there was an incomplete rotation of the ascending colon to the right side, allowing the kidney to move freely. In one case, the kidney moved into a retroperitoneal pocket of the mesocolon. The 6 cases with a lateral passage for the kidney were symptom-free at follow-up (30-80 months), but in the 7th case the patient's kidney quickly loosened and she underwent an open reoperation, after which she was symptom-free. Conclusion: Our series demonstrates that good results can be achieved with a transperitoneal laparoscopic approach, but also indicates that there is a common pathogenic mechanism with incomplete rotation of the ascending colon that can be corrected during surgery, which might contribute to the good results.
